1. Register with My Aged Care

Start by creating an account with My Aged Care, the government gateway for aged care services. This ensures you’re connected to the right support options.

2. Assessment of Care Needs

An Aged Care Assessment Team (ACAT) or regional assessor evaluates your care requirements, including mobility, daily activities, and health needs.

3. CARE PLAN AND PACKAGE APPROVAL

Based on the assessment, a Support at Home Package is recommended, determining the level of care, approved hours, and funding (Levels 1–8).

4. CHOOSE YOUR PROVIDER

Compare local in-home care providers and select one that best matches your needs, preferences, and lifestyle.

5. RECEIVE TAILORED SUPPORT

Start receiving services at home — from personal care, domestic assistance, and nursing support to social activities, restorative care, or end-of-life care pathways.

6. MONITOR AND ADJUST AS NEEDED
